Colostomy as a stigma in clients at their productive age and the role of a nurse in co-operation with these clients

I chose ``Colostomy as a stigma in clients in their productive age and the task of the nurse in cooperation with these clients{\crqq} as the topic of my bachelor thesis for several reasons. On the one hand, I think that colostomy is talked about a lot all the time, but if really want to obtain any literature, very little is available. And a large number of colostomy patients call for more sources of information. And on the other hand, I find this topic close for personal reasons: there are colostomy patients in my own family and among my close friends. Whether they have colostomy after a tumorous disease during Crohn disease, they still suffer from very similar problems that lead to a certain stigmatization that they both consciously and unconsciously bring into their mind. This is why this topic occurred to me. It is necessary to survey what stigma is, colostomy patients{\crq} opinion of stigma and the way nurses view stigma. In my opinion, this is an unusual combination of two problems that are widely discussed, yet neither dealt with nor described in any way. At the present time, which is a time of mass media and stress, an individual person could hardly think that he or she does not know certain aspects of life that a certain percentage of the population meets with, and in many a case does not even know about this question. People think and believe that they are prepared for everything, that they are balanced and can deal with all problems. When, however, a problem touches their health, it is often an aspect that they do not even think about very much and consider it as a natural part of their everyday life, they start to be literally helpless and anxious, and this escalates depending on how large an interference in their health it is. If you say the term stomy, a lot of people hesitate and have no idea what it could be. It is of course no rule but we have to get aware of the thing that a large part of the population deal with this problem. It is therefore important to conduct education and to pay attention to the information of the broad population, while placing emphasis on understanding stomy patients{\crq} everyday troubles. The term stigma, as I have found out to my surprise, is a completely unknown notion. This is despite the fact that stigma and stigmatising factors may occur in a broader range of diseases than the stomy mentioned. The objective of the thesis is then to shed light on the terms of stigma a colostomy. And to find out the most frequent stigmatising factors in this disease and to find a solution to this problem.
The difficulty of the work of nurses at standard level internal wards

This Bachelor's Thesis deals with the issue of the difficulty, or level of physical and psychological demand, of nurses at standard level internal hospital wards. The internal ward is one of the basic wards included in hospital care. It is here that patients with internal organ ailments are hospitalized, and treatment lies in a conservative approach. The work of nurses at a standard level internal ward consists in providing quality health and nursing care. The nurse knows all the illnesses that occur here as well as their treatment and complications. She is also expected to be empathic and patient. Because she is in daily communication with doctors, patients, and their families, she must know all the principles of assertive communication. The job description of the nurse includes satisfying the needs of patients, participating in diagnostic operations, fulfilling the orders of the doctor, and preparing nursing documentation. The sister does not always witness a happy ending to each treatment. All these factors place a burden on the entire organism from both a psychological and physical standpoint. The objective of this Bachelor's Thesis is to map out the difficulty of the work of nurses at a standard level internal ward. To achieve this objective, qualitative research investigation was carried out using a semi-structured interview. The research base was formed by nurses from the internal wards of the hospital in České Budějovice (Nemocnice České Budějovice a. s.) and the Jindřichův Hradec hospital (Nemocnice Jindřichův Hradec a. s.). The interviews with the respondents were transcribed into case reports and further processed into categorization tables. The results clearly show that the work of nurses at standard level internal wards is very difficult and demanding from both the physical and the psychological side. The case reports and tables also show another piece of important information ? signs of ?burn-out? syndrome and the influence of stress on the work of the nurses, relaxation techniques of the nurses, satisfaction with financial compensation, and much more. On the basis of the results, an informational brochure for nurses was drawn up that includes relaxation techniques such as autogenous training, meditation, and yoga.
Urinary Bladder Evacuaton by means of CIC (clean intermittent catheterization)

Urinary Bladder Evacuaton by means of CIC (clean intermittent catheterization). This Thesis deals with application of intermittent catheterization in treatment practice. This techique is used both, for clients with neurogenic disorder and clients with non {--} neurogenic disorder of urination. The Thesis was aimed at examination to what extent clients of a urodynamics advisory service are informed about the technique of urinary bladder drainage, whether the clients are able to perform urinary bladder drainage by means of the CIC technique themselves in normal living conditions, how clients using CIC are limited in normal home activities and to compare occurrense of urinary tract infections between clients with CIC and those with pernament catheter. A quantitative research survey method was applied to obtain the data for the Thesis. The data were collected by means of a questionnaire designed for clients of the urodynamics advisory service of the Hospital České Budějovice a.s. 100 questionnaires were distributed from January to March 2009. The return was 87 %. 87 questionnaires were returned, 5 of which had to be rejected for incomplete data. 82 complete filled in questionnaires (100 %) were used for the data processing. Four hypotheses were set. Hypothesis 1, clients are informed on the CIC technique, has been confirmed. Hypothesis 2, clients using CIC technique to drain their bladders have problems with urine evacuation in normal lives, has not been confirmed. Hypothesis 3, clients using the CIC technique are not limited in normal home activities has been confirmed. Hypothesis 4, clients with CIC have lower occurence of urinary tract infections than clients with permanent catheters has been confirmed. The research results will be provided to the Urology Department of Hospital České Budějovice a.s. to improve the care of clients using the CIC technique.
Demands and risks of nursing work providing nursing care to agressive patients.

This bachelor work is trying to analyse how demanding and risky nursing is when aggressive clients are treated. The theoretical part describes the origin of aggressive behaviour, types of aggression and situations which generate aggressive behaviour. All nursing parts of the problem are described here such as a preparation for the arrival of a disturbed client, his reception to the hospital and the main tasks of nurses in the psychiatric ward. Some other methods how to calm down and restrict the patient are also analysed. A quantitative approach, questioning and semistructured interviews were used to collect all data in this work. The interview consisted of 28 questions and each respondent could answer in his words. The data were collected in mental hospitals in Havlíčkův Brod and Prague-Bohnice and in a psychiatric ward of the hospital in České Budějovice. Nurses who work with aggressive patients answered to the questions. Some results are demonstrated in graphs and tables and some answers are thoroughly analysed. Five objectives were stated at the beginning of the research and all of them were fulfilled. Eight research issues were set and they were gradually responded and the hypotheses were specified. The work reached the conclusion that nursing staff provides a very demanding and risky job when they treat aggressive patients. The nurses know what to do if the patients attack them to avoid injuries of nursing staff , the aggressive patient himself and other patients. They can use a restrictive methods in such situations. On the other hand, they would praise a training course in self-defence and communication with aggressive clients as well as more men staff in the wards.
